The Process

Three steps. About two minutes.

The entire process is completed online, in a single sitting.

1

Sign in to your patient portal

Select the button above and sign in with the same MyChart username and password you already use for your hospital or clinic. Your login takes place on your hospital's own website — we never see your password.

2

Review and approve

Your hospital shows you exactly what is being requested for your legal matter. You review the list and choose "Allow access." That approval is your authorization — no signature, notary, or paperwork.

3

Your records are delivered

The records transfer directly and securely from your hospital to the team supporting your case, typically within seconds. No further action is required on your part.

Your Privacy

  • Encrypted in transit — your records travel directly from your hospital to your legal team
  • Your password stays with your hospital; we never see or store it
  • Shared only with your law firm and the team supporting your case — never sold, never used for marketing

Who is Strategic Litigation Partner LP?

Strategic Litigation Partner LP (SLP) is a litigation support company that provides services such as document review, e-discovery, records retrieval, and expert or consulting support to law firms and other litigants. SLP is not a law firm, does not provide legal advice or representation, and is not your attorney. Records you authorize through this portal are provided to the law firm handling your matter.

What is MyChart, and do I have an account?

MyChart is the patient website used by most major hospitals and clinics. If you have ever received an email or text from your doctor's office inviting you to "view your test results online," you likely have an account. If you are unsure, contact your clinic — or call our office and we will help you confirm.

Is it safe to sign in to my hospital account through this site?

Yes. When you begin, you are taken to your hospital's own official sign-in page — the same one you would use normally. We never see your username or password. Your hospital verifies your identity, shows you what is being requested, and releases records only after you approve.

What records will my legal team receive?

The categories listed on your hospital's approval screen — your health record as the hospital holds it: clinical notes and documents, lab and test results, conditions, medications, allergies, immunizations, visits, procedures and surgeries, implants, vital signs, care plans and social history. You will see the complete list before you approve anything, and nothing is shared until you do.

I was treated at more than one hospital. What should I do?

Repeat the process once for each hospital or health system where you received care — each takes about two minutes. If you are unsure which of your providers are covered, contact our office and we will assist you.

Does this cost me anything?

No. There is no charge to you for sharing your records this way — and it spares you the copying fees and long waits that come with traditional records requests.

Can I change my mind after approving?

Yes. You can revoke access at any time from your MyChart account settings, and the authorization also expires on its own. You stay in control from start to finish.

Ready to begin?

A few minutes now can save weeks of waiting.
