Stop sharing or delete my records
You can ask us at any time to stop using the medical records you shared and to delete the copies we hold. There is no form to fill in — talking to a person is faster, and we have to confirm your identity before changing anything anyway.
How to ask
Call (713) 574-6689 and tell us you want to stop sharing or delete your records.
Have your name and date of birth ready. If you know the reference number for your case, that helps us find you faster.
What happens then
- We confirm it is really you
- A member of our records team verifies your identity first, so that nobody else can stop or delete your records by pretending to be you.
- We stop using them
- We stop using your records for anything other than what the law requires.
- We delete what we are allowed to delete
- Where a court order, court rule, or a record-retention obligation requires us to keep something, we cannot lawfully destroy it — and we will tell you plainly what is being kept and why.
- The app already has no standing access
- MyMedRecords collected your records once, at the moment you approved it. It cannot go back to your hospital for more unless you authorize it again.
